Ages and Stages Is The Gold Standard For Developmental Screening Of Your Child

We are asking every caregiver to fill it out prior to their child’s well child visit.

This is simple to fill out as it is an online form.

Instructions:

Click on the link above:

1. Will take you to the ASQ home page

2. Fill in Child’s DOB

3. For weeks premature, enter 0 unless prior to 37 to 42 weeks: then enter the number of weeks

4. Select screening date (always the top one with today’s date)

5. SKIP LIST OTHERS ASSISTING WITH COMPLETION OF THE QUESTIONNAIRE

6. Click Submit

7. Click Enter your result

8. Start the ASQ questionnaire

The questionnaire takes about 10 to 15 minutes to fill out.
